Student and Instructor Application Forms

Licence renewal applications can be made using our licensing portal:

Please note:

Use the ‘BWK Licence Form‘
After completing the form, you must review it before a submission button is presented
You will need to know the correct email address of your club’s instructor to complete the form
For renewals, a blank image can be uploaded in place of a photograph
